Summary
Introduction
In gynecologic surgery, laparotomy is the preferred approach when full access to the pelvis and/or upper abdomen are desired, particularly when pelvic adhesions are suspected or exploration of the retroperitoneal space may be required. Pelvic or abdominal masses, suspected carcinoma, a history of prior abdominal surgery, and suspected retroperitoneal pathology are all indications for considering laparotomy over laparoscopy. The abdomen and pelvis may be accessed through either a longitudinal or transverse approach. The choice of incision is based on the area in which surgical exposure is desired while reducing patient discomfort and postoperative complications. Proper surgical technique and the judicious use of self-retaining retractors with awareness of abdominal and pelvic neuroanatomy can minimize postoperative complications. Incision closure technique has changed dramatically since the advent of longer-lasting absorbable suture material, resulting in fewer incisional complications, improved patient satisfaction, and reduced hospitalization.
Scope of the Problem
The widespread development of minimally invasive and robotic techniques for gynecologic and general surgery in urban and teaching hospitals has made the laparotomy a less commonly performed procedure. Availability of open and vaginal hysterectomies has diminished to the point that American residency programs are concerned about appropriate laparotomy experience for each resident. Gynecologic oncology services, long a source of laparotomies and hysterectomies for ovarian cancer cytoreduction and radical hysterectomies for cervical cancer, have reduced the use of laparotomy in favor of robotic procedures. The “open” abdominal hysterectomy is at risk of becoming a “lost art,” with some residency graduates unable to confidently perform the procedure in general practice. In a recent survey of gynecologic oncology fellowship directors, half of the respondents reported that their incoming fellows could not independently perform an abdominal hysterectomy, and 40 percent could not recognize anatomy and tissue planes. As the number of open teaching cases declines, it becomes more important that the surgeon maintain a conscious awareness of proper surgical technique and practices that optimize patient safety. Application of good surgical technique with reusable instruments can make the open hysterectomy a cost-effective alternative to laparoscopic and robotic procedures.
Prevention of Venous Thromboembolic Events
A number of factors must be considered when choosing the appropriate venous thromboembolic prophylaxis regimen for each patient. Patient age, expected length of time of surgery, history of prior thromboembolism, and diagnosis of inherited thrombophilia all influence the risk of postoperative thromboembolism.

We describe the investigation of two temporally coincident illness clusters involving salmonella and Staphylococcus aureus in two states. Cases were defined as gastrointestinal illness following two meal events. Investigators interviewed ill persons. Stool, food and environmental samples underwent pathogen testing. Alabama: Eighty cases were identified. Median time from meal to illness was 5·8 h. Salmonella Heidelberg was identified from 27 of 28 stool specimens tested, and coagulase-positive S. aureus was isolated from three of 16 ill persons. Environmental investigation indicated that food handling deficiencies occurred. Colorado: Seven cases were identified. Median time from meal to illness was 4·5 h. Five persons were hospitalised, four of whom were admitted to the intensive care unit. Salmonella Heidelberg was identified in six of seven stool specimens and coagulase-positive S. aureus in three of six tested. No single food item was implicated in either outbreak. These two outbreaks were linked to infection with Salmonella Heidelberg, but additional factors, such as dual aetiology that included S. aureus or the dose of salmonella ingested may have contributed to the short incubation periods and high illness severity. The outbreaks underscore the importance of measures to prevent foodborne illness through appropriate washing, handling, preparation and storage of food.

In North America, terrestrial records of biodiversity and climate change that span Marine Oxygen Isotope Stage (MIS) 5 are rare. Where found, they provide insight into how the coupling of the ocean–atmosphere system is manifested in biotic and environmental records and how the biosphere responds to climate change. In 2010–2011, construction at Ziegler Reservoir near Snowmass Village, Colorado (USA) revealed a nearly continuous, lacustrine/wetland sedimentary sequence that preserved evidence of past plant communities between ~140 and 55 ka, including all of MIS 5. At an elevation of 2705 m, the Ziegler Reservoir fossil site also contained thousands of well-preserved bones of late Pleistocene megafauna, including mastodons, mammoths, ground sloths, horses, camels, deer, bison, black bear, coyotes, and bighorn sheep. In addition, the site contained more than 26,000 bones from at least 30 species of small animals including salamanders, otters, muskrats, minks, rabbits, beavers, frogs, lizards, snakes, fish, and birds. The combination of macro- and micro-vertebrates, invertebrates, terrestrial and aquatic plant macrofossils, a detailed pollen record, and a robust, directly dated stratigraphic framework shows that high-elevation ecosystems in the Rocky Mountains of Colorado are climatically sensitive and varied dramatically throughout MIS 5.

Ten years ago, research in second language acquisition manifested an array of doubts about the potential of major disciplinary paradigms for providing a comprehensive theory of second language acquisition. Linguistic theory and psychological theory alike were viewed as only marginally useful either for stimulating fruitful research or for undergirding classroom practices in second language teaching. But the skepticism of the 70s had its virtues. We had outgrown the childishness of seeking clear-cut distinctions and final answers. Consequently, we probed deeper and wider, beyond the confines of “established” disciplinary models to seek new worlds of possibilities, yet with philosophical lifelines firmly secured in the collective expertise of centuries of the study of human behaviour.

INTRODUCTION
In the century spanning the mid-1880s to the mid-1980s, the language teaching profession was involved in what many pedagogical experts would call a search. That search was for a single, ideal method, generalizable across widely varying audiences, that would successfully teach students a foreign language in the classroom. Historical accounts of the profession tend, therefore, to describe a succession of methods, each of which is more or less discarded in due course as a new method takes its place. I will comment on “the changing winds and shifting sands” (Marckwardt, 1972, p. 5) of that history momentarily; but first, we should try to understand what we mean by method.
What is a method? More than three decades ago, Edward Anthony (1963) gave us a definition that has quite admirably withstood the test of time. His concept of method was the second of three hierarchical elements, namely, approach, method, and technique. An approach, according to Anthony, was a set of assumptions dealing with the nature of language, learning, and teaching. Method was defined as an overall plan for systematic presentation of language based on a selected approach. It followed that techniques were specific classroom activities consistent with a method, and therefore in harmony with an approach as well.
Some disagreement over Anthony's definition can occasionally be found in the literature. For Richards and Rodgers (1986), method was an umbrella term to capture redefined approaches, designs, and procedures.

Introduction
Two main themes have dominated empirical research in community ecology for the last several decades. On the one hand, the enormously successful experiments of British plant ecologists and of intertidal ecologists such as Connell (1961a, b) and Paine (1966) stimulated many ecologists to perform controlled, replicated, manipulative field studies to investigate direct and indirect interactions. On the other hand, the insightful observations of Hutchinson (1957) and MacArthur (1958) stimulated many other ecologists to explore the relationships between patterns of community organization and the mechanistic processes that produce them. These two approaches have tended to diverge in the kinds of organisms and the spatial and temporal scales studied. The experimentalists tended to work with terrestrial plants, freshwater fishes and amphibians, and intertidal marine organisms, to manipulate on small spatial scales, and to study ongoing, proximate ecological processes. In contrast, those studying patterns of community organization tended to study terrestrial reptiles, birds, and mammals, to do comparative studies over geographic spatial scales, and to be concerned with evolutionary, as well as ecological, processes and timescales. Most studies of community-level patterns and processes have focused on two phenomena: the organization of food webs (e.g., Cohen et al., 1993; Polis & Strong, 1996; papers in Special Feature section of Ecology 69, pp. 1647–1676) and the structure of ‘guilds’ (sets of functionally similar and closely related species; e.g., Cody & Diamond, 1975; Diamond & Case, 1986; Simberloff & Dayan, 1991).

It was only about a year ago, in a reckless moment of shelf-purging, that I reluctantly placed my original offset copy (circa 1975) of The Good Language Learner into the paper-recycling bin. Many of us fondly remember that big orange-covered prepublication volume, dog-eared, battered, and stuck together with tape. Now, almost two decades after the first publication of The Good Language Learner (Ontario Institute for Studies in Education, 1978) this reissue appears as part of Multilingual Matters' “Modern Languages in Practice” series. It is the first time this series has featured a reprint of “classic” (p. vii) volumes of interest to foreign language researchers and teachers.
